Bully-Free at NCUJHS

 

Bullying is a serious problem and we are doing all we can to prevent it, but we need your help. The bullying form is a way for you to give us information about when bullying occurs and who is involved. This information is used to identify bullies and victims so that they can get the help they need. By reporting bullying, you are helping to make NCUJHS a bully-free, comfortable place where students can learn without fear of being bullied. Thanks for helping with this important mission.

How do I use the bullying report form (on the other side of this paper)?

 

·         Each student will have a form in his/her planner for easy use. New forms will be available in the Guidance office and TA’s.

·         On the name line, you may write your name or ANONYMOUS which means you prefer to make the report without giving us your name. If you do give us your name, it will help us because we may need to contact you for more information. We will not share your name with anyone.  Whether you give us your name or not, please report any bullying. If you witness bullying, it is better to make a report anonymously than to not report at all. Thanks.

·         Write the names of the people involved in the situation.

·         Write the date and time when the situation occurred.

·         Write the location where the incident occurred.

·         On the blank lines, write exactly what you saw and heard. Remember to report only the facts. We are looking for responsible reporting.

·         The completed form should be placed in the bully referral box in the guidance office.

·         The box will be checked often by Administration and Guidance, and bullying situations will be dealt with as soon as possible.

·         If you need help filling out the form, just ask a guidance counselor or any other staff person you feel comfortable with.
